Responsibilities of a Hospice RN / RN Case Manager:
  1. Report directly to the Administrator, Executive Director, or Patient Care Manager Senior and maintain primary control and professional management of each patient.
  2. Act as the primary liaison between physician, patient/family, and hospice team.
  3. Provide direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable caseload while building trusting and compassionate relationships.
  4. Empower patient’s caregivers by educating them on care for their loved one.
  5. Manage and collaborate with an interdisciplinary group including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
  6. Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
  7. Maintain all assigned patient clinical records.
  8. Participate in company quality assessment and performance improvement programs.
Qualifications for Hospice Registered Nurse / RN Case Manager:
  1. Compassionate, kind, and empathetic towards patients and families.
  2. Comfortable providing care wherever the patient calls “home”.
  3. Demonstrate flexibility and composure during crises.
  4. RN license in the requested state (or eligibility to obtain).
  5. At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology is strongly preferred.
  6. Additional preferred experience includes hospital Staff RN, Med Surg, PACU, ER, critical care, and other acute care roles.
  7. Valid driver’s license and automobile insurance.
  8. Current CPR certification.
