AccentCare is seeking a Hospice Registered Nurse (RN) to provide compassionate, patient-centered care in Weld/Larimer Counties. You will work within an interdisciplinary team, promoting dignity and comfort for patients and families in their most delicate moments.

Qualifications

• Registered Nurse (RN) license in the state of practice.
• At least one year of nursing experience.
• Reliable transportation, valid driver’s license, and appropriate auto insurance.

Responsabilités

• Provide individualized nursing care that maximizes comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
• Perform comprehensive assessments—physical, emotional, social, and spiritual—to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately to reflect patient needs and progress.
• Participate in oncall or admission rotations as needed.
• Engage in ongoing learning and professional development.

Overview

Hospice Registered Nurse (RN)

Pay: $40 - $45/hour , based on experience

Schedule: PRN as needed 4-6 shifts of availability per month to include some on call support

Coverage Area: Weld / Larimer Counties
