Day Kimball Health is hiring for a Hospice Registered Nurse (RN) for the Homecare Part Time Day Shift 32-Hours in Putnam, CT.

Job Summary:

As the Hospice Registered Nurse (RN), you will provide comprehensive nursing services to Hospice and Palliative Care patients and families. You will report to the Associate Director of Hospice/Palliative Care for policy and fiscal matters and to the Hospice Clinical Supervisor for clinical matters. You will comply with all DKHC personnel policies as an employee of Day Kimball HomeCare.

Responsibilities:
  • Provide skilled nursing care to hospice and palliative care patients in their homes, including admission and discharge, maintaining complete patient records.
  • Teach, demonstrate, supervise, and evaluate care given by peers, HHAs, family members, and significant others.
  • Assess patients systematically based on age and developmental level, addressing physical, psychosocial, emotional, cultural, spiritual, and discharge needs.
  • Communicate effectively, ensuring completeness and accuracy of documentation.
  • Evaluate and adjust the care plan as needed.
  • Operate patient care equipment skillfully, including venipuncture for IV medication administration.
  • Develop and implement individualized care plans.
  • Recognize unsafe and life-threatening situations and intervene appropriately.
Education and Experience Requirements:
  • Graduate of an accredited nursing school.
  • Valid Connecticut RN license.
  • At least two years of recent medical/acute or critical care nursing experience preferred.
  • Achieve Home Health Care specific competencies within the department's timeframe.
  • Valid Connecticut Driver’s License and proof of insurance.
  • Understanding of hospice philosophy and goals preferred; hospice training is encouraged and to be completed on the job if not already obtained.
  • Within 2 years of hire, pursue and maintain certification in Hospice & Palliative Care.
