Job summary

Shalom Hospice in Nashville, TN seeks a Clinical Hospice Liaison for an in-person role with a 7 on/7 off schedule. The RN will assess potential patients for hospice eligibility and coordinate a smooth transition to hospice care, educating patients, families, and providers on hospice benefits.

Responsibilities include collaborating with physicians and discharge planners, documenting all assessments, and serving as a clinical resource to marketing and business development. Travel may be required.

Qualifications

• Licensed RN with hospice/palliative care or acute care experience.

Position Summary:

The Clinical Hospice Liaison serves as the clinical expert in the pre-admission process, assessing potential patients for hospice eligibility and coordinating a smooth and compassionate transition to hospice care.
