Job summary

Gentiva Hospice is seeking an experienced Hospice Clinical Liaison to support intake, referrals, and admissions in the McAllen, Texas area. You will educate families about hospice care, coordinate with hospitals, physicians, and case managers, and serve as a liaison to ensure smooth handoffs with the care team.

You will build strong referral networks, visit hospitals daily, and help expand community awareness of hospice services while adhering to regulatory requirements.

Qualifications

• Licensed nurse with hospice experience, 3–5 years required
• BSN with hospice experience 5+ years preferred
• CPR certification required
• Current RN license to practice inState of employment

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians and other healthcare professionals
• Assess patient eligibility for hospice services by reviewing records and communicating with physicians/families
• Educate patients and families on hospice care and what it includes
• Coordinate intake and admission process, ensuring legal, clinical, and payer requirements are met
• Liaise between referral sources and hospice care team for seamless handoffs
• Make daily visits to hospitals and facilities to stay accessible to discharge planners and physicians
• Build and maintain relationships with referral partners and identify outreach opportunities

In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services.

About You

Education

• Licensed Nurse with three to five years of hospice experience required.
• BSN with five or more years of hospice experience preferred.

Experience

• Minimum of three years of hospice experience required.
• Prior experience as a discharge planner, case manager, or educator preferred.

Licenses and Certifications

• Current RN license to practice nursing in the state of employment.
• CPR certification.
• Valid driver’s license and automobile insurance.

Skills And Knowledge

• Strong knowledge of hospice care and the unique needs of patients and families.
• Familiarity with community resources and referral networks.
• Excellent patient assessment and documentation skills.
• Ability to supervise staff, coordinate services, and manage programs effectively.
• Strong communication and interpersonal skills with sensitivity to diverse backgrounds.
