Compass Healthcare Consulting & Placement is conducting a search for a Hospice RN Case Manager, RN Registered Nurse, to make visits to Hospice patients in Hospital/facility or at home. Competitive Salary & Excellent Benefits. Qualified candidates must have NYS RN. Position requires travel to a hospital facility located in Manhattan as well as, patients homes located in Upper & Lower Manhattan. Qualified candidates will have prior experience in medical-surgical, community health, home health, and/or hospice, palliative care, oncology nursing, preferred.

Hospice RN Case Manager

Field-based position visiting patients to perform both physical and psychosocial assessments and electronically document visit occurrences. Patient visits will be made in both community and facility settings. You will develop and implement nursing plans of care and make recommendations for care based on patients' needs. The schedule includes working one weekend per month and functioning in a backup On Call capacity 2-3 times per month. Car and escort service available for backup On Call shifts.

Qualifications

  • Associates/Diploma from an accredited school of Nursing; BSN preferred
  • 2+ years' experience in a med/surg, community health, and/or oncology nursing setting preferred
  • NYS RN license
  • NYS Driver's license
