Job summary

Three Oaks Hospice is seeking a compassionate RN to provide skilled nursing care in patients' homes in the Hurst area. You will coordinate with an interdisciplinary team, educate families, and support end-of-life care planning.

The role requires an RN license, CPR certification, and at least two years of clinical RN experience; hospice/home health experience is preferred. Strong communication and ethics are essential for success.

Qualifications

• RN with current state license and CPR certification.
• Two years RN experience in a clinical care setting; hospice/home health preferred.
• Graduate of an accredited nursing program with an Associate or Bachelor's degree.
• Proficient in using a computer; Microsoft Office experience preferred.
• Demonstrate strong ethics, communication and organizational skills.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial and environmental needs.
• Coordinate care with the interdisciplinary team and refer to services as needed.
• Educate patients and families about disease process, self-care and end-of-life care.
• Provide training to staff and participate in care planning.
• Manage on-call responsibilities and document progress toward goals.

Position Summary

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence coordinate care with the interdisciplinary team patientfamily and referring agency; and assumes the responsibility for coordination of care

Essential Duties

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical functional psychosocial and environmental needs and ADLs ie risk for grief culturalspiritual verbal non verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Consulting with and educating the patientfamily regarding disease process self care techniques end of life care nutrition and dietary needs
• Providing training to other staff as needed
• Initiating appropriate preventive and rehabilitative nursing procedures
• Preparing clinical and progress notes that demonstrate progress toward established goals
• Coordinating all patientfamily services and prioritization of needs with the members of the IDT
• Use of case management approach and referring to other services as needed
• Informing physician and other personnel of changes in the patients needs and outcomes of intervention while evaluating patientfamily response to care
• Determining scope and frequency of services needed based on acuity and patientfamily needs
• Assessing ability of the caregiver to meet the patients immediate needs upon admission and throughout care
• Evaluating own needs for support and using identified systems to meet the need
• Communicating information using current process and technology available to the organization
• Applying specific criteria for admission and re certification to hospice care to establish appropriate levels of care and the patients eligibility
• Perform on call responsibilities and provide on call service to patientsfamilies as assigned
• Other duties as assigned

Qualifications

• Graduate of an accredited Associate or bachelors degree in Nursing
• Current state license as an RN and current state drivers license
• Two years experience as an RN in a clinical care setting; previous hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice ability to cope with family emotional stress and tolerance of individual lifestyles
• Strong communication and organizational skills and high standard of ethics and compliance
