Job summary

Amedisys is seeking a Hospice Registered Nurse for per diem shifts in Virginia to provide home-based care. You’ll perform assessments, develop care plans, and coordinate with the care team to ensure comfort and quality for patients at home.

Competitive pay: $38-$42 per hour plus mileage, with flexible hours and on-call responsibilities. Ideal candidates hold an active RN license, CPR, a valid driver’s license, and at least one year of nursing experience or participate in the RN new graduate

Qualifications

• Active RN license valid in the state(s) where you work.
• CPR certification required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes and revises the care plan in coordination with staff.
• Regularly communicates patient progress to the clinical manager and care team.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of nursing experience preferred. If less than one year of nursing experience, you can be considered for the RN new graduate program.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
