Qualifications

Responsibilities

• Completes initial and ongoing comprehensive assessments of hospice patients.
• Determines appropriate levels of care and eligibility for hospice.
• Develops and revises care plans with patient and family.
• Coordinates with interdisciplinary team to ensure continuity of care.
• Provides patient and family education on caregiving and pain management.
• On-call duties as defined by policy.
