Job summary

Valor Hospice in Houston, TX is seeking a registered nurse to provide comprehensive hospice care in patients' homes. You will coordinate with the interdisciplinary team, assess needs, and implement care plans to ensure comfort and quality of life for patients and families.

The position requires an active RN license, a valid driver's license, and at least two years of nursing experience in a clinical setting, with hospice experience preferred.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state RN license and driver's license.
• Two years' nursing experience in a clinical care setting; hospice experience preferred.
• Strong interpersonal skills; good written and spoken English.
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Conduct initial and ongoing assessments of patient needs and functioning.
• Develop and document the plan of care to ensure quality and continuity of service.
• Educate patients and families about disease process, end-of-life care, and self-care techniques.
• Coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency.
• Provide on-call responsibilities and respond to patient needs after hours.
• Communicate changes in patient condition to physicians and other team members.
