Autumn Hospice in Worcester, MA is hiring an RN Case Manager to assess patient needs, develop care plans, and coordinate end-of-life care with the interdisciplinary team. This role emphasizes compassionate nursing, family counseling, and ongoing evaluation of nursing care.
The ideal candidate has an Associate Degree in Nursing, an active RN license, and at least two years of nursing experience (hospice preferred). Travel is required, and a competitive benefits package is offered.

The RN Case Manager's duties include, but are not limited to:
Complete an initial assessment of patients and their families to determine hospice care needs.
Regularly re-evaluate patients' nursing needs.
Use assessment data to develop and update individualized care plans.
Collaborate with the interdisciplinary healthcare team, patients, and families to ensure coordinated, continuous care.
Counsel patients and families regarding end-of-life care and support.
Initiate appropriate preventive and palliative nursing interventions.
Communicate with physicians regarding patient needs and report changes in condition.
Supervise Hospice Aides.
