Ascend Hospice & Palliative Care VA is hiring an experienced RN Case Manager in Midlothian, VA to join our compassionate hospice team. This role involves on-call coverage, case management visits and coordinating patient care to ensure smooth, continuous services.

You will assess patient and family needs, develop care plans, collaborate with the healthcare team, and support end-of-life decision-making. A state RN license and willingness to travel are required, with a $5,000 sign-on bonus.

Competenze

• Must have a State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid Driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Mansioni

• Complete initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate the patient’s nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated.
• Counsels the patient and family in meeting end-of-life needs.
• Initiates appropriate preventative and palliative nursing procedures.
