Job summary

Ascend Hospice & Palliative Care MA is hiring an RN Case Manager to support on-call coverage, case management, and patient visits. You will assess patients and families, develop care plans, and collaborate with the healthcare team to ensure continuous, coordinated end-of-life care.

Requirements include an active RN license in Massachusetts, at least 2 years of nursing experience (hospice preferred), a valid driver's license, ability to travel, and strong verbal and written communication skills.

Qualifications

• 2 years' nursing experience; hospice experience preferred.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel; strong verbal and written communication skills.

Responsibilities

• Complete initial assessments of patients and families to determine hospice care needs.
• Re-evaluate patients' nursing needs regularly and adjust care plans.
• Collaborate with the healthcare team to ensure continuous and well-coordinated care.
• Counsel patients and families on end-of-life needs and preferences.
• Initiate appropriate nursing procedures and communicate changes to physicians.
• Supervise hospice aides and coordinate visits.

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Supervises the Hospice Aides
