Job summary

Compassus is seeking a Hospice Registered Nurse / RN Case Manager in Southaven, MS to provide compassionate end‑of‑life care in patients’ homes and communities. You will collaborate with a multidisciplinary team to coordinate care, manage symptoms, and support families with education and resources.

The role requires an active RN license, at least 2 years of nursing experience (hospice preferred), CPR/BCLS, and a valid driver's license.

Qualifications

• Current RN license in the state of practice.
• Minimum of 2 years of nursing experience; hospice experience preferred.
• Current CPR and BCLS certification.
• Valid driver’s license and reliable transportation.
• Able to build trusting, compassionate relationships with patients and families.

Responsibilities

• Provide hospice nursing care to patients in home or designated settings.
• Coordinate and manage patient care plans with the care team.
• Educate patients and families on symptom management and care needs.
• Document care activities and outcomes in the EMR system.
