Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join our team and lead patient-centered hospice care in the home setting. You will assess patient and family needs, coordinate interdisciplinary care, and support the care team to ensure the highest quality outcomes.

The role requires 50% travel, a current RN license, and 1 year of clinical experience, with opportunities for advancement and professional development within a collaborative environment.

Qualifications

• Graduate of an accredited nursing program (Certificate, Diploma, ADN, or BSN).
• Current State RN license.
• Minimum 1 year clinical care experience.
• Hospice or home health experience preferred.
• Willingness to travel ~50%.
• Must be able to lift up to 50 pounds and move equipment.
• Valid driver’s license and insurance; reliable transportation.

Responsibilities

• Ensures activities align with Elara Caring’s board, executive team, and Hospice leadership.
• Assesses hospice patients and families to identify physical, psychosocial, and environmental needs.
• Assesses patient needs across physical, psychological, social, and spiritual factors.
• Communicates significant findings to Clinical Supervisor, physician, facility, and other involved personnel.
• Counsels and includes patient, facility, and family in the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain management to staff in alternate care settings.
• Provides bereavement support at time of death and conducts bereavement assessment.

As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
• Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.
• Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
• Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).
• Provides appropriate support at time of death and perform bereavement assessment.

What is Required?

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse RN
• 1 year of experience in a clinical care setting
• Experience in a hospice or home health environment is preferred
• 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid driver’s license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
