Job summary

Plan A Health in Georgia is seeking a compassionate Hospice Registered Nurse to provide end-of-life care in patients' homes and facilities. You will perform comprehensive assessments and develop individualized care plans.

You will administer medications and treatments to manage symptoms, collaborate with physicians, social workers, chaplains, aides, and families, educate patients on hospice services, document all care, and participate in on-call rotations and interdisciplinary team meetings.

Qualifications
