Job summary

Delta Health System in Mississippi is seeking a dedicated Registered Nurse to join our patient-focused team. The RN will plan, coordinate, and document care for an assigned patient caseload and act as a liaison to referral sources for Home Health services.

Requires current Mississippi RN license, BLS certification, and one year of acute care nursing experience; home health experience is preferred. A valid driver's license and auto insurance are also required.

Qualifications

• Plan, coordinate, and document care for an assigned caseload of patients.
• Serve as liaison to referral sources by handling Home Health referrals and facilitating HomeCare services.
• Current RN licensure in the State of Mississippi.
• Current BLS certification.
• Credentials in specialty areas (e.g., phlebotomy, IV, ET).
• One year of acute care nursing experience.
• Prior experience in home health preferred.
• Valid Mississippi driver's license with current auto insurance and reliable vehicle.
