Job summary

St. Croix Hospice is seeking a Registered Dietitian to join the clinical care team, delivering nutrition care to patients in their homes as part of an interdisciplinary approach. You will plan, implement, and assess dietetic care aligned with each patient’s Plan of Care.

The role emphasizes counseling families and coordinating with physicians, with a focus on improving quality of life and dignity in end-of-life care. A Missouri RD credential and a master’s degree are required.

Qualifications

• Missouri Registered Dietitian Credential.
• Master’s degree in dietitian studies from an accredited institution.
• Ability to pass background, consumer report, and drug test.
• Valid State Driver’s License and access to a reliable vehicle; driving record acceptable.

Responsibilities

• Develops and implements a nutrition plan of care based on a comprehensive assessment as part of the patient’s total plan of care.
• Provides counseling to individuals and families in the home setting in collaboration with the physician and interdisciplinary team.
• Reevaluates nutritional care through home follow-up visits or phone contacts.
• Documents dietary history and nutrition care data in patient records and discharge summaries.
• Attends interdisciplinary group meetings to coordinate care.
• Complies with organizational policies and preserves patient confidentiality and safety.
• Participates in quality improvement activities and performs other duties as assigned.
