Job summary

St. Croix Hospice is seeking a Registered Dietitian to join our clinical care team. The RD will plan and deliver nutrition education, support patients and families, and contribute to the interdisciplinary approach in a home-care setting.

The ideal candidate holds a Wisconsin RD credential, a master’s in dietetic studies, and can navigate arranging care, documentation, and follow-up with patients in their homes or by phone. A valid driver's license is required.

Qualifications

• Wisconsin Registered Dietitian Credential required.
• Master’s degree in dietetic studies from an accredited program.
• Ability to pass background checks and drug test.
• Valid State Driver’s License and reliable vehicle.

Responsibilities

• Develops and implements a nutritional plan of care based on comprehensive assessment.
• Provides counseling to individuals and families in the home setting.
• Communicates dietary history and care data through written records.
• Performs home follow-up via in-person visits or telephone.
• Participates in interdisciplinary group meetings and quality improvement.
