Offer Based on Years of Experience
The Quality Improvement Specialist is responsible for monitoring, analyzing, and conducting in-depth reviews of key quality metrics foundational to AccentCare's ability to deliver high-quality, safe care to our patients and achieve progress on the Hospice Quality Reporting Program. Ensures compliance with state, federal, and accreditation standards. This position is a member of the quality management team and provides expertise in quality improvement, performance improvement, and change management. They promote commitment to corporate and subsidiary standards, policies, and procedures while teaching, coaching, and consulting on measure development, methodology, goal setting, data analysis (including root cause and risk assessment), problem solving, corrective action, and evaluating the effectiveness of actions taken. The role guides leaders through project work, facilitation, consultation, coaching, skill-building/training, and reporting in a complex community-based healthcare environment. They work closely with teams in Quality, Clinical Operations, and Compliance departments, supporting root cause analysis, safety event facilitation, and coaching teams on implementing change.
