Job summary

Continuum Hospice & Palliative Care California is seeking a Program Director to oversee our hospice program in Capitola, CA. The role provides leadership for operations, growth, external relationships, financial management, and quality care delivery.

You will ensure compliance with hospice regulations, Medicare/Medicaid matters, and oversee day-to-day local office activities under senior management guidance. This is a hands-on leadership role focused on improving patient care and family support.

Qualifications

• Bachelor's degree in nursing, health care, or business admin preferred.
• Minimum of two years in health care management or hospice.
• Two years of hospice experience with Medicare/Medicaid knowledge.
• Experience in operations or sales leadership.
• Proven ability to build community relationships.

Responsibilities

• Forge strong connections with physicians, hospitals, and facilities to extend reach.
• Build the team with orientation, training, and evaluation.
• Organize and supervise administrative functions of the hospice program.
• Maintain compliance with applicable laws and regulations.
• Lead day-to-day management of the hospice program.
• Implement policies and procedures approved by the governing body.
