Job summary

Frontpoint Health is seeking a compassionate Hospice LVN to join our home-care team serving Rowlett, Rockwall and Forney, TX. This PRN role focuses on providing skilled nursing care in patients' homes under RN or physician supervision.

You'll assess health status, administer medications, monitor vitals, document care in EMR, and collaborate with families and an interdisciplinary team to ensure high-quality, compliant care in a supportive environment.

Job Description:

We are seeking a skilled and compassionate Hospice LVN to join our team. The LVN will be responsible for providing nursing care to patients in their homes, under the direction of a registered nurse or physician. The ideal candidate will have experience in hospice care, excellent communication skills, and a passion for helping others.

Responsibilities:

• Assess patients' health status and develop care plans
• Administer medications and treatments as prescribed by a physician
• Monitor patients' vital signs and report any changes to the RN or physician
• Provide wound care and other nursing interventions as needed
• Document patient care and progress in electronic medical records
• Communicate with patients, families, and other healthcare providers as needed
• Participate in interdisciplinary team meetings and case conferences
• Adhere to all state and federal regulations and agency policies and procedures

Requirements:

• Current LVN license in the state of Texas
• Minimum of one year of experience in Hospice care
• Excellent communication and interpersonal skills

• Valid driver's license and reliable transportation
• Ability to pass a background check and drug screening
