Job summary

Heartland Hospice is seeking a dedicated Hospice Physician to deliver compassionate, goal-directed care to patients while upholding the highest medical standards. You will collaborate with interdisciplinary teams, oversee regulatory compliance, and guide quality improvement initiatives to enhance patient outcomes.

This is a flex/per diem role with opportunities to lead and educate staff, participate in policy development, and build partnerships with community providers as part of our mission to

Qualifications

• Doctorate degree in allopathic or osteopathic medicine from an accredited medical school.
• Board certification in ABMS or AOA-BOS, preferably primary care.
• Current and unrestricted medical license in the practicing state.
• Valid National Provider Identifier (NPI) and DEA registration for controlled substances.

Responsibilities

• Certify and recertify hospice eligibility per Medicare and regs.
• Supervise face-to-face visits and home consultations for complex cases.
• Provide 24/7/365 on-call coverage coordination with hospice physicians.
• Maintain timely medical documentation for regulatory reviews.
• Participate in IDG meetings to coordinate care and medication management.
• Monitor pharmacy utilization and appropriate prescribing.
• Collaborate on policy development, QAPI initiatives.
• Serve as community liaison to increase hospice visibility.
• Support educational programs for staff and partners.
