Job summary

VitalCaring Group is seeking a Physical Therapy Assistant to provide skilled services under a licensed Physical Therapist, supporting hospice patients and families with comfort, function, and quality of life.

You will work with a compassionate team, participate in care planning, and help patients progress through exercises and mobility activities, contributing to meaningful patient outcomes daily.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program with an associate degree in science or applied science.
• Current PTA certification or licensure as required by state law.
• Strong verbal, written, and organizational skills.
• Current CPR certification.

Responsibilities

• Deliver physical therapy interventions as directed by a licensed Physical Therapist and in accordance with the physician’s plan of care and the state Physical Therapy Practice Act.
• Lead patients through active and passive therapeutic exercises, gait training, and mobility activities.
• Utilize physical agents such as heat, cold, ultrasound, and massage in alignment with state practice guidelines.
• Document clinical notes and updates in the patient’s record and communicate with the team.
• Attend home care team meetings and participate in IDG and quality improvement activities.
