VitalCaring Group is seeking a dedicated Hospice Physical Therapist to plan, organize, and deliver comfort-focused therapy in patients’ homes. You will guide patients and families on safe mobility, education, and supportive techniques to promote dignity at the end of life.

Qualifications

• Bachelor’s or master’s degree in physical therapy from an accredited program.
• Licensed to practice as a physical therapist in the state of practice.
• Two years of relevant experience preferred; home health or community-based experience helpful.
• Strong verbal, written, and organizational skills.
• Current CPR certification and valid driver's license with insured vehicle.

Responsibilities

• Provide physical therapy services according to physician’s plan of care and state policy.
• Assess functional status and participate in the total plan of care development.
• Evaluate home environment for safety and comfort.
• Instruct patients and families on therapy techniques and device use.
• Train, supervise, and coordinate with PTAs as appropriate.
• Prepare clinical notes, progress and discharge summaries.
• Participate in discharge planning and IDG meetings.
