Job summary

VitalCaring Group is seeking a Physical Therapist to plan, organize, and deliver therapy services focused on comfort, function, and quality of life for patients at home and hospice settings. You will guide patients and caregivers through supportive techniques, mobility, and education to promote dignity at end of life.

The role emphasizes compassionate care, collaboration with an interdisciplinary team, and adherence to state practice acts.

Qualifications

• Must hold a degree in physical therapy and be licensed.
• Two years of relevant experience preferred; home health or community-based experience helpful.
• Strong verbal, written, and organizational skills.

Responsibilities

• Provide PT services according to physician's plan and state PT Act.
• Assess and develop the total plan of care with patient goals.
• Evaluate home environment to ensure safety and comfort.
• Assess strength, mobility, gait, ROM, and transfer abilities.
• Instruct patients and families on therapy techniques and equipment use.
• Identify needs and report changes to the physician.
• Initiate therapy program and educate caregivers and team.
