OnePoint Patient Care in Tulsa, OK is hiring a full-time Pharmacist to join our closed-door, non-retail hospice pharmacy team. You will review prescriptions, ensure safety, and collaborate with physicians and nurses to optimize therapy.

Competenze

• Bachelor’s Degree in Pharmacy, PharmD preferred.
• Active pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines and regulatory standards (CMS, DEA, FDA).
• Proficiency with EMR and pharmacy information systems.
• Strong clinical judgement and effective written and verbal communication.
• Position posted for a minimum of 7 business days.

Mansioni

• Receive and evaluate prescriptions for drug appropriateness, correct dosage, interactions.
• Collaborate with physicians, nurses, and other healthcare professionals for clinical support.
• Maintain up-to-date patient profiles, including medical history and medications.
• Provide clinical recommendations to physicians and hospice nurses.
• Ensure compliance with state and federal regulatory requirements.
• Oversee narcotics room and automation; supervise IV meds prep and dispensing systems.
• Perform other pharmacy-related tasks as assigned.
