Job description

The Team Manager is the leader of the Patient Care Team, responsible for supervising, evaluating, and coordinating the interdisciplinary team members. They ensure continuity of care from admission to discharge or transfer to bereavement, serve as a patient advocate, and coordinate with community social service and healthcare providers involved in patient care. They also maintain patient records from intake through discharge or transfer.

Qualifications:

  • Reliable transportation with appropriate license and insurance
  • Private home telephone
  • Knowledge of primary medical and nursing care principles, with familiarity in oncologic nursing and palliative care
  • Understanding of interpersonal dynamics within healthcare teams and knowledge of dying and bereavement processes
  • Working knowledge of VITAS information system (Vx) and management reports
  • At least two years of supervisory experience in healthcare or equivalent

Education:

  • Bachelor's degree preferred
  • Valid RN license in the state of practice
