Position Overview
The Hospice/Palliative RN provides compassionate and skilled nursing care to patients with life-limiting illnesses, ensuring comfort and dignity in the patient’s final stages of life. This role involves working closely with families to manage pain, address symptoms, and provide emotional and psychological support, all while collaborating with other health care providers to deliver holistic, patient-centered care in the home setting.
Schedule: 3-days a week
Work Location: On the road in assigned territory, including Saugus and surrounding communities.
Position Type: Part Time
Compensation: $50-55 per hour
Essential Duties & Responsibilities
- Conduct comprehensive assessments of patients’ physical, emotional, and psychological needs.
- Develop and implement individualized care plans in collaboration with the patient’s family, physician, and interdisciplinary team.
- Administer pain management and symptom relief medications, ensuring comfort and quality of life.
- Provide direct nursing care, including wound care, catheter care, and monitoring vital signs.
- Educate patients and families on disease progression, comfort care techniques, medication management, and end-of-life planning.
- Offer emotional support and counseling to both patients and their families, addressing concerns about grief and loss.
- Coordinate care with the hospice team, including social workers, chaplains, and volunteers, to ensure comprehensive support.
- Maintain accurate patient records, documenting assessments, care interventions, and outcomes.
- Ensure compliance with hospice care regulations and guidelines, including patient confidentiality and legal standards.
Qualifications
- Current RN (Registered Nurse) license with hospice or palliative care certification preferred.
- Minimum of 2 years of nursing experience, with experience in hospice or end-of-life care strongly preferred.
- Knowledge of pain and symptom management techniques, including palliative care practices.
- Excellent communication and interpersonal skills to engage with patients and families.
- Ability to work independently, prioritize tasks, and manage a flexible schedule.
- Strong organizational skills and attention to detail.
- Compassionate, empathetic, and patient-focused demeanor.
