Job summary

Anchor Health is seeking a Medical Social Worker (MSW) to support hospice and palliative care patients and their families. You will assess social and emotional needs, coordinate care plans, and assist with practical arrangements to ease illness-related burdens.

The role integrates with the hospice interdisciplinary team to provide ongoing support, planning, and advocacy for patients and caregivers facing serious illness. Experience with advance care planning is preferred.

Qualifications
