Job summary

Main Line Health, Inc. is seeking a Social Worker for Hospice and Palliative Care to provide psychosocial support to patients and families facing life-limiting illness. You will coordinate needs with the IDG and contribute to care planning, education, and resource linkage.

The role requires a Master’s Degree in Social Work, PA license, CPR, and a driver’s license; hospice experience is preferred. This is a full-time daytime position with benefits and opportunities for involvement in committees

Qualifications

• One year social work experience in health care setting preferred.
• Hospice or palliative care experience preferred.
• Master's Degree in Social Work from an accredited program.

Responsibilities

• Provide psychosocial support and counseling for patients and families.
• Coordinate psychosocial and community resource needs with the IDG.
• Address social, emotional, financial, and spiritual needs to improve quality of life.
