Job summary

VitalCaring Group is seeking a Hospice Occupational Therapist to plan, organize, and deliver therapeutic services that maintain comfort and dignity for end-of-life patients. You will work within a values-driven team to provide skilled interventions and compassionate care, guided by trust, capability, and proactive support.

The position emphasizes in-home patient care, collaboration with interdisciplinary teams, and ongoing training and career growth opportunities within a supportive environment.

Qualifications

• Graduate of an accredited occupational therapy program per AOTA/AMA standards.
• Registered via National Registration Examination (NBCOT).
• Minimum of two years clinical OT experience.
• Current CPR certification.

Responsibilities

• Evaluate patient functional status including muscle function, endurance, coordination, self-care and work capacity.
• Assess home environment for hazards and necessary adaptive equipment.
• Develop and implement individualized treatment plans and goals.
• Teach activities and retrain daily living skills per policy.
• Fabricate splints and train on adaptive equipment use.
• Provide training for prosthetic/orthotic devices as indicated.
• Educate family and caregivers on treatment plan and needs.
• Support discharge planning and care transitions.
