Job summary

Three Oaks Hospice is seeking a passionate On-Call Registered Nurse (RN) to join our team in delivering high-quality hospice care in patients’ homes. You will administer skilled nursing care, coordinate with an interdisciplinary team, families, and referring agencies to ensure seamless care delivery.

Requirements include an accredited nursing degree, current RN license, valid driver’s license, CPR certification, and 2+ years of RN experience (hospice/home health preferred).

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience is preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

Responsibilities

• Administer skilled nursing care for clients in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency.
• Coordinate care and support to ensure smooth, consistent patient outcomes.
• Maintain documentation, monitor patient status, and respond to changes in condition.
