Job summary

Sage Hospice Primary & Palliative Care in Tempe, AZ is seeking a compassionate Registered Nurse to join our on-call PRN team. You will provide skilled nursing care to patients in their homes and coordinate with a multidisciplinary team to ensure comfort and quality of life.

We value clinical excellence, empathy for patients and families, and the ability to respond promptly to on-call needs. Prior hospice or home health experience is preferred; CPR certification is required.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice and ability to cope with family emotional stress.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Provide on-call RN coverage as part of the PRN hospice team.
