Job summary

Agape Hospice Care seeks an On-Call Registered Nurse (RN) to join our hospice team in Alpharetta, GA. The role involves providing skilled nursing care to patients in their homes, coordinating with the care team, families, and referring agencies, and ensuring smooth continuity of services.

The ideal candidate holds an RN license, CPR certification, and at least two years of clinical RN experience (hospice/home health preferred). Strong communication and computer skills are essential for success.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and current state driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.

Responsibilities

We are looking for a passionate On-Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.
