Concho Hearts Hospice PRN On Call RN
Area: Texarkana, Texas

Our team of Nurses, Aides, Chaplains, Social Workers, Volunteers, and Doctors provide symptom management, and patient and family support while coping with an end-of-life illness. Job Summary: Primary functions are to administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient, family, and referring agency; and assume the responsibility for coordination of care.

RN Responsibilities:

  1. Preparing and adjusting patient care plans as needed.
  2. Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
  3. Maintaining and updating medical charts, patient files, and health records.

RN Job Qualifications:

  1. Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  2. Licensure: Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Drivers License.
  3. Experience: Two years experience as a Registered Nurse in a clinical care setting preferred, hospice preferred.
  4. Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
  5. Specialized skills preferred in palliation and end-of-life. Proof of current CPR. Must read, write and comprehend English.

Transportation:

  1. Reliable transportation.
  2. Valid and current auto liability insurance.

Environmental and Working Conditions:

  1. Works in patients' homes in various conditions, possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule.
  2. Ability to travel locally.
  3. Some exposure to unpleasant weather.
  4. PRN emergency call.

Physical and Mental Effort:

  1. Prolonged standing and walking are required, with the ability to lift up to 50 lbs and move patients.
  2. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition.
  3. Meet patient/family individualized psycho-social needs.
