Job summary

AccentCare in Texas is seeking a Hospice Registered Nurse Case Manager (RN) to join a dedicated interdisciplinary team. You will provide compassionate nursing care, partner with families and physicians, and work at the top of your license with support from a mission-driven organization.

This role covers Killeen area with Full Time M-F schedule. You will perform comprehensive assessments, educate patients and caregivers, coordinate across disciplines, and participate in on-call rotations while

Qualifications

• Valid RN license in the state of practice.
• At least one year of nursing experience.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide individualized nursing care to maximize comfort and quality of life.
• Collaborate with patients, families, physicians, and the care team to develop and implement care plans.
• Perform comprehensive assessments to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care accurately reflecting patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.

Hospice Registered Nurse (RN) Qualifications

• Registered Nurse (RN) license in the state of practice

• Reliable transportation, valid driver's license, and appropriate auto insurance
