Job summary

CP04 Carilion Franklin Memorial Hospital seeks a dedicated RN Hospice Nurse to deliver compassionate end-of-life care. This full-time position requires evening/night shifts and on-call availability from Monday to Thursday, along with home visits with mileage reimbursement.

The ideal candidate will have at least one year of experience in hospice or related fields, a valid Virginia RN license, and commitment to patient advocacy and interdisciplinary collaboration.

Qualifications

  • Graduate of an accredited registered nurse program.
  • Minimum 1 year experience in hospice, home health, oncology, or medical-surgical nursing.
  • Currently licensed as a Registered Nurse in Virginia or a compact state.
  • Valid driver’s license and personal vehicle for home visits.
  • Must maintain AHA BLS-HCP certification.

Job description

Job Overview

The RN Hospice Nurse will provide end‑of‑life nursing care, coordinate symptom management, and collaborate with the interdisciplinary team to deliver compassionate care in various settings. This full‑time, evening/night shift position requires on‑call availability Mon‑Thurs and covers home visits with mileage reimbursement.


Responsibilities


  • Complete admissions, coordinate care, and conduct follow‑up visits.

  • Review patient eligibility and collaborate with the interdisciplinary team, Medical Director, and attending physician to develop care plans.

  • Perform comprehensive assessments and develop individualized plans of care.

  • Administer treatments and medications accurately and document care in electronic medical records.

  • Advocate for patients, supporting their rights and psychosocial, cultural, and emotional needs.

  • Participate in interdisciplinary team meetings and collaborate across care teams.

  • Contribute to performance improvement initiatives and support positive patient outcomes.

  • Utilize clinical expertise to manage dynamic patient conditions and improve care efficiency, outcomes, and resource utilization.

  • Serve as a role model, providing support to staff, students, and team members.

  • Use problem‑solving skills to address patient care and operational challenges.

  • Maintain strong critical‑thinking and organizational skills to manage competing priorities.

  • Use electronic systems to support communication, documentation, and continuity of care.

  • Collaborate with billing to ensure authorization requirements and patient financial responsibilities are addressed.

  • Demonstrate flexibility in scheduling and prioritization of patient care needs.

  • Ensure compliance with hospice policies, procedures, and regulatory requirements.


Qualifications

  • Currently licensed as a Registered Nurse in the state of Virginia or a compact state.

  • Valid driver’s license, automobile insurance, and personal vehicle for home visits.

  • Must maintain AHA BLS‑HCP certification.

  • Keyboarding skills for automated computer systems and electronic medical record use.


Compensation

$76,689.60 – $115,044.80 per year (excluding benefits or pension plan). Pay varies with qualifications, credentials, education, and experience.
