Job summary

Three Oaks Hospice is seeking a Nurse Practitioner to join our palliative care team. You will work with a physician and the IDG to deliver compassionate, evidence-based care to patients and families in the hospice setting.

You will perform visits, help develop care plans, and educate patients and staff about disease progression and end-of-life options. Strong communication and collaboration with a multidisciplinary team are essential.

Qualifications

• Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current state RN and APN license and CPR certification.
• Minimum 2 years’ experience as an APN in clinical care setting preferred; hospice and home health preferred.
• Certification in a specialist area preferred; (e.g., Hospice and Palliative Nursing (CHPN) or Geriatrics)
• Excellent observation, verbal and written communication skill, problem solving skills, nursing skills per competency checklist.
• Reliable transportation, valid driver’s license, and current auto liability insurance coverage.

Responsibilities

• Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
• Consults with and assists in the IDG’s development of patient/family plan of care to assure quality care is provided.
• Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
• Performs and documents visits to patient/family based on plan of care and patient/family need.
• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
• Consults with and provides education to other IDG members on a prn basis for clinical issues relating to palliation.
• Actively participates in the IDG. Communicates effectively, timely, and appropriately at all times.
• Informs physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.
