Job summary

UnitedHealth Group's HighPoint Hospice is seeking a Hospice Nurse Practitioner to provide palliative and supportive care to patients and families in Gallatin, TN. You will collaborate with the Hospice Medical Director and interdisciplinary team to develop and implement care plans.

Ideal candidates hold NP or CNS certification, current CPR, and at least 5 years of nursing experience, with license in good standing in your state.

Qualifications

Active unrestricted license in the state of residence.
Current certification as a Nurse Practitioner or Clinical Nurse Specialist in the state of practice.
Current certification by a national body such as ANCC, NBCHPN, or AANP.
Current CPR certification.
5+ years of nursing experience.
Valid driver's license and access to dependable transportation.

Responsibilities

Face-to-face hospice visits within prescribed time limits and report findings to the Hospice Medical Director.
Consult with the Hospice Medical Director and inform primary physicians of recommended services.
Coordinate plan of care; document goals, interventions, and patient/family response.
Instruct and supervise patients/families in self-care as appropriate.
Maintain accurate clinical notes and report patient needs to Administrator and Hospice Medical Director.
