Job summary

Community Healthcare of Texas seeks a Nurse Practitioner to provide hospice care in collaboration with the Chief Medical Director and Hospice Team. Qualifications

• Licensed Advanced Practice Nurse in Texas.
• At least 2 years of experience in geriatrics or hospice.
• Excellent organizational skills.
• Proficient computer skills; Microsoft Office experience required.

Responsibilities

• Conduct Face-to-Face visits to assess patients' hospice eligibility.
• Complete required documentation to support eligibility and regulatory communication.
• Communicate to RN Case Manager any patient needs requiring medical intervention.
• Coordinate visits and scheduling with the Patient Care Manager.
• Maintain compliance with all regulatory and accrediting agencies.
• Demonstrate understanding of the philosophy, mission, and service standards and provide excellent customer service.
