Ideal candidates hold an accredited NP degree and CT APRN license, with prior hospice or palliative care experience highly preferred. Flexible, per-diem shifts support work-life balance while maintaining high-quality patient care.

Formación

• APRN credential required.
• Board-certified in family or adult-gerontology care.
• Experience with hospice/geriatrics preferred.

Responsabilidades

• Provide face-to-face visits for hospice patients.
• Participate in on-call coverage (minimum 4 weeknights/month).
• Collaborate with IDT to coordinate care across settings.
