Qualifications

• Licensed Nurse Practitioner with collaborating physician supervision.
• Experience with terminal illness and hospice care.
• Knowledge of Medicare/Medicaid hospice regulations.

Responsibilities

• Provides medical care and manages terminal illness under supervision of a collaborating physician.
• Performs history and physical examinations and documents findings in medical records.
• Develops individualized plan of care, prescribes medications and coordinates hospice care services.
• Reviews hospice eligibility and completes face-to-face visits as required by hospice regulations.
• Communicates with hospice interdisciplinary team and coordinates care with physicians and other practitioners.
• Educates patients and families about care plans, symptoms, and end-of-life issues while ensuring patient safety.
