Job summary

Good Shepherd Community Care, a community-based hospice and palliative care provider, seeks an experienced Hospice Nurse Practitioner for a full-time role in the Newton, MA area. You will join an expanding interdisciplinary team to deliver compassionate care to hospice patients in the community setting.

Responsibilities include initial and ongoing hospice eligibility visits, symptom management, and timely documentation.

Qualifications

  • Master’s NP degree required and current Massachusetts NP license.
  • Minimum 2 years nursing experience with at least 1 year in hospice, palliative care, or oncology.
  • Familiarity with Medicare Part A hospice regulations and eligibility requirements.

Responsibilities

  • Complete face-to-face visits for initial and ongoing hospice eligibility and assess symptom burden.
  • Communicate significant visit findings to the designated hospice physician in a timely fashion.
  • Engage with the hospice interdisciplinary team and provide symptom management recommendations.
  • Prescribe medications for hospice patients as necessary.
  • Document visits in the EMR after adequate training.
  • Schedule and manage weekly hours: Monday to Friday, 9 am–5 pm.
