Mid Michigan Home Health and Hospice is seeking a compassionate Hospice Nurse Practitioner to join our interdisciplinary team. You will provide high-quality palliative and end-of-life care to patients in their homes and other settings, focusing on comfort and dignity.

You will collaborate with physicians, nurses, social workers and spiritual care staff; educate families; manage symptom control; and document in EMR while participating in on-call rotations.

Qualifications

• Current and unrestricted NP license in the state.
• Active or eligible for DEA license.
• Minimum 1 year hospice/palliative or geriatrics experience preferred.
• Strong clinical assessment & communication skills.
• Valid driver’s license and reliable transportation.

Responsibilities

Position Summary

We are seeking a skilled and compassionate Hospice Nurse Practitioner (NP) to join our interdisciplinary care team. The ideal candidate will provide high-quality palliative and end‑of‑life care to patients in their homes, assisted living facilities, and other care settings. This role focuses on symptom management, patient education, and collaboration with the care team to ensure comfort and dignity for our patients.

Key Responsibilities

• Conduct comprehensive assessments and develop individualized hospice care plans.
• Provide palliative symptom management and coordinate with the hospice team for ongoing patient needs.
• Collaborate closely with physicians, nurses, social workers, and spiritual care providers.
• Educate patients, families, and caregivers on disease progression, comfort measures, and care plans.
• Document all patient encounters accurately and timely in the EMR.
• Participate in interdisciplinary team meetings, on‑call rotation, and staff training as needed.
