Job summary

Lakes Region Visiting Nurse Association in Meredith, NH seeks a Hospice Registered Nurse to act as Case Manager for hospice clients, coordinating with the Interdisciplinary Team to create personal care plans. Full-Time with competitive salary and benefits.

You'll perform initial assessments, develop care plans with the IDG, and ensure timely documentation and communication with physicians and the team. Hospice experience is beneficial, and training is provided.

Qualifications

  • Graduate of an accredited school of nursing.
  • 1-2 years of recent patient centered care/nursing experience.
  • Current state licensure, CPR certification, and a valid driver’s license.
  • Bachelor's degree in nursing with one year of hospice care experience preferred.
  • Strong observation, communication, and problem-solving skills.

Responsibilities

  • Completes an initial assessment of patient and family to determine any immediate needs and brings them to the Interdisciplinary Team (IDG) immediately to address.
  • Completes the HIS within two days of the election of hospice and again at discharge, in accordance with the hospice CoP’s (Conditions of Participation).
  • Completes nursing comprehensive assessment on admission and election of hospice.
  • Coordinates with the IDG to ensure that all disciplines complete their assessments within five days of the election of hospice, in accordance with the hospice CoP’s.
  • Develops a care plan with the support of the IDG that establishes goals based on the client’s diagnosis, physical, emotional, and spiritual needs.
  • Prepares clinical notes and communicates with the primary physician/ hospice medical director every fourteen days.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition.
  • Obtains/receives physicians orders as required.
  • Communicates with the entire interdisciplinary team to coordinate/ update the client’s plan of care.
  • Ensures that all required documentation is done and submitted on time in compliance with the hospice CoP’s.

Position Summary:

The Hospice Registered Nurse acts as the Case Manager for their hospice clients. They work closely with all the members of the Interdisciplinary Team (IDG) to create a highly personal and individualized plan of care for their hospice clients. They are responsible for organizing and directing the services for hospice clients, their families and caregivers based on identified needs and client/ family goals.

The Hospice Registered Nurse is experienced in nursing, with an emphasis on community health, as well as having experience with hospice, palliative care and /or end of life care.

We offer Full-Time hours with excellent salary and benefit packages. While hospice experience is beneficial, we provide training for the right individuals. Strong clinical support and educational resources are also available.
