Role Description

This full-time, on-site Hospice Nurse role is based in Columbia, SC and focuses on providing compassionate end-of-life care for patients and support for their families. The Hospice Nurse will conduct comprehensive patient assessments, administer medications and treatments, and manage symptoms to promote comfort and quality of life. Daily responsibilities include coordinating care plans with physicians and interdisciplinary team members, documenting patient status, and educating patients and caregivers about hospice services and home care routines. The role also involves participating in case conferences, advocating for patient preferences, and responding to changes in condition with timely clinical interventions. The Hospice Nurse is expected to follow regulatory and organizational standards while fostering a respectful, supportive environment for patients and families.

Qualifications

• Clinical expertise in Nursing and Medicine relevant to hospice and end-of-life care.
• Strong skills in Palliative Care, including pain and symptom management and psychosocial support.
• Experience in Home Care and Direct Patient Care, delivering bedside services in residential settings.
• Current Registered Nurse (RN) license in the state of South Carolina.
• Previous hospice, home health, or palliative care experience preferred.
• Ability to collaborate within an interdisciplinary team and communicate clearly with patients, families, and colleagues.
• Capacity to work independently, manage time effectively, and respond to urgent patient needs.
• Knowledge of hospice regulations, documentation standards, and infection control practices.
• BLS certification and a valid driver's license with reliable transportation for patient visits.
