Job summary

Qualifications
- Must hold a current Georgia license for Advanced Practice Nursing.
- Certified as Gerontologic NP or Family/Adult NP by respective boards.
- Two years in management of Medicare patients; home-based care experience preferred.
Responsibilities
- Collaborates with a physician to deliver end-of-life and palliative care.
- Serves as a consultant, advocating for hospice programs and educating healthcare professionals.
