UnitedHealth Group is seeking a Per Diem Hospice Music Therapist to join a dedicated interdisciplinary home health and hospice care team. This role focuses on emotional, psychosocial, and physical well-being through individualized music therapy interventions in a patient-centered plan of care.

Responsibilities include assessing needs, developing care plans, providing interventions, coordinating with clinicians and families, and documenting sessions.

Qualifications

• Bachelor's degree in Music Therapy from an accredited program.
• Board Certification in Music Therapy (MT-BC).
• 1+ years of experience in music therapy, healthcare, hospice, or related setting.
• 1+ years of experience with chronic, serious, or end-of-life conditions.
• Demonstrated solid interpersonal, communication, and therapeutic skills.
• Driver's License and access to reliable transportation.

Responsibilities

• Assess patient needs and develop individualized music therapy care plans in coordination with the interdisciplinary team
• Provide music therapy interventions to support symptom management, emotional well-being, and quality of life
• Collaborate with clinical staff, caregivers, and families to integrate music therapy into overall patient care plans
• Maintain accurate and timely documentation of therapy sessions, patient responses, and care plan updates
• Participate in interdisciplinary team meetings and contribute to care coordination and patient outcomes
