Job summary

BAYADA Home Health Care is seeking an energetic Medical Social Worker (MSW) for a full-time hospice visits role in Norwich, VT. You will serve clients across Windsor, Orange and Grafton counties, primarily between St.

Johnsbury and Ludlow, VT, collaborating with a multi-disciplinary team to support home-based care. Ideal candidates hold an MSW with CSWE accreditation and have at least 2 years of community-based experience.

Qualifications

• Master of Social Work (MSW) with CSWE accreditation.
• Minimum 2 years of community-based experience preferred.
• English communication skills required; ability to travel to cases.
• EMR knowledge or experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Provide clinical social evaluations and plan appropriate interventions.
• Identify community resource referrals for patient and family needs.
• Advocate for clients and navigate the patient through community systems.
• Participate in the total care plan with the multidisciplinary team.
• Develop and maintain contracts with agencies as resources.
• Educate families on community resources to support health and safety.
• Document observations and evaluations in the EMR on a touch pad tablet.
